Registration forms are available below.  Please download or print the forms. When completed they should be returned to the Church office along with payment for the registration; fees are listed below.

No child will ever be denied participation for financial reasons.  If you have a concern, please contact Chris Potter-Wroblewski, our Children’s Religious Education Coordinator.
