Registration forms are available below.  Please download or print the forms. When completed they should be returned to the Church office along with payment for the registration; fees are listed below.

Registration for Children ages 3 through 5th Grade- English

No child will ever be denied participation for financial reasons.  If you have a concern, please contact the church office.

 **Catholic Youth Ministry Registration**

Catholic Youth Ministry provides for the religious education and spiritual development needs of all youth in the parish in grades 6 thru 12. Annual registration is required even if you have been a CYM member in previous years. This is needed so that we know who is participating and so that we have current information on each youth and their parents. This is especially helpful for effective communication.

CYM regstration includes the following:

1. Complete the CYM Membership Form, one side for youth registration and one side for parent registration. Both pages must be completed in full. Parent signature required.
2. Submit a signed copy of the Youth Code of Conduct. Youth and parent signatures required.
3. Payment of the annual fee which is due upon registration. The fee is $50.00 per student up to $150.00 maximum per family.
